Therapist vs Psychiatrist: What’s the Difference & Which Do You Need?

By DocSana · October 8, 2026 · 5 min read

The single most confusing question in mental health care — answered plainly, with Austin-specific guidance on cost and how to choose.

The short version

Therapist

  • Provides talk therapy (counseling)
  • Degrees: LCSW, LMFT, LPC, PsyD, PhD
  • Cannot prescribe medication
  • Sessions: usually 45–60 min, weekly
  • Austin: $150–$250/session

Psychiatrist

  • Medical doctor (MD/DO) for mental health
  • Can prescribe medication
  • Diagnoses mental health conditions
  • Visits: 15–30 min follow-ups
  • Austin: $300–$500 first visit

When to start with a therapist

For most people, most of the time: start here. Therapy alone effectively treats mild-to-moderate anxiety, depression, grief, relationship issues, stress, and trauma. A good therapist will tell you honestly if they think medication might help — and refer you to a psychiatrist.

When to see a psychiatrist

Many people see both

This is the most common arrangement for moderate-to-severe conditions: a therapist for weekly talk therapy, a psychiatrist for medication management every few weeks or months. They don’t have to be in the same practice — though it helps if they can coordinate.

What about psychologists, NPs, and counselors?

Quick decoder: psychologists (PhD/PsyD) do therapy and testing, usually not medication. Psychiatric nurse practitioners (PMHNP) can prescribe in Texas and often have sooner availability than psychiatrists. LCSWs, LMFTs, and LPCs are all licensed therapists with different training emphases — for most talk therapy, any of them can be excellent.

Frequently asked questions

Can a therapist prescribe medication?

No. In Texas, only psychiatrists (MD/DO), psychiatric nurse practitioners, and certain other medical prescribers can prescribe psychiatric medication. If your therapist thinks medication would help, they’ll refer you.

Do I need a referral to see a psychiatrist?

Usually not — most psychiatrists in private practice accept self-referrals. Some insurance plans require one, so check your plan. On DocSana you can contact providers directly.
